How To Fill Out ON-LINE Meal Application
Make sure you’re at the on-line web page: www.tulsaschools.org/meal_applications
Once the form is displayed, on PART 1: Does the household have SNAP, TANF? If not, leave the field blank.
Part 2: Enter all students attending TPS (Middle and High Schoolers). Do not enter the Student ID, it will self-populate after clicking on the Lookup button. If you get error message, continue.
Part 3: Enter last 4 of SSN, First, last name, address, phone, email account. If they entered a SNAP number in Part 1, they do not have to put in the last 4 of SSN.
Part 4: Enter ALL family names not listed in Part 2. Enter income amount and the frequency of income. Everything else under Part 4 is optional. However, if the parent wants a Refusal of Benefits, they need to check the box.
Part 5: Select the Accept option twice. After clicking on the radio button twice a digital signature box will come up with the person that is filling out the on-line application will appear. Select OK, then on the right, select the Go button. That’s it!
